About these early intervention templates
Early intervention templates give a child development team one shared set of documents: a developmental case history, a multidisciplinary assessment, a family-centred team goal plan, session notes, progress reports, a home programme, a parent coaching plan, and a transition summary. Each one opens in your browser without sign-up and downloads as a clean PDF.

A young child may see a speech therapist, an OT, a physiotherapist and a special educator in one month. Separate forms mean the family repeats its history four times. These templates are written for the whole team, with the family's routines at the centre.
Each template is structure only and leaves out milestone ages, age comparisons, and licensed test items. Every section has a short how-to, and a fictional sample entry where an example helps.
Questions
What documents does an early intervention team need?
Most teams use a case history, a developmental assessment, a goal plan agreed with the family, session notes, progress reports, a home programme or coaching plan, and a transition summary. This set has one template for each.
Are these templates suitable for child development centres in India?
Yes. They are written in Indian English, with fields for home languages, grandparents and other daytime carers, and preschool settings. Rename any heading to match your centre's format before downloading.
Do the templates include sample entries?
Yes. Each page shows a fictional sample entry for most sections, using initials only. Summary and impression sections have no sample, because the clinician writes those from their own findings.
Do the templates include developmental milestones or test items?
No. They give structure and guidance only. If your team uses a standardised developmental tool, record its name and results exactly as its manual reports them, and keep the protocol in your records.
Can I fill these in online and download a PDF?
Yes. Each template opens in the free editor in your browser without sign-up. Drafts stay in your browser, and you can download a clean PDF to print or share with the family.
Who should write each document?
The family fills in the case history, ideally with a team member. The assessment, goal plan, progress report, and transition summary are team documents with one coordinator. The therapist who ran the session writes the session note.
Does healhab include these as built-in starters?
healhab's built-in starters cover speech, occupational, and behaviour therapy. A child development centre builds its own templates in Template Studio, and its services are configured during setup.
Write every report inside your clinic system.
Keep these as your clinic's own templates, write from reviewed session notes, publish branded PDFs, and keep families updated.
