The essentials

What is in Templates & tools?

Templates by therapy and by document, a free report writer, small tools and a goal bank.

  • Templates by therapy: speech therapy, occupational therapy, ABA and behaviour therapy, early intervention, physiotherapy and rehabilitation.
  • Templates by document: assessment reports, case history and intake forms, treatment plans, SOAP and session notes, progress reports, home programmes, discharge summaries and data sheets.
  • The online report writer: open any template or a blank page and download a PDF.
  • Tools and goal bank: age calculators, a SMART goal builder, a session data tracker and example goals by skill area.

The four parts link to each other, so you can move from a plan to a note to a report without starting again.

Why are the templates structure only?

Clinics use different assessments, so templates give the frame and leave tests and conclusions to you.

Indian clinics use a wide mix of assessments: play-based observation in one centre, a bought published test in another, a hospital case sheet in a third. A template built around one test would suit almost nobody, and copying licensed test items onto a public page is not ours to do.

So each template gives the frame: details, history, methods, observations, findings, recommendations, goals and sign-off. Where a test belongs, you record its name and the result as its manual reports it. The impression and recommendations stay with the clinician who saw the person.

How do I fill in a template online?

Pick a template, select "Fill it in online", type into each section, then download the PDF.

  • Choose a template by therapy or by document type and read the section guidance on its page.
  • Select "Fill it in online" to open it in the report writer.
  • Type into each section, delete the prompts you do not need, and add or remove table rows.
  • Download the PDF to print, sign, or share through your clinic's usual channel.

You do not need an account. The steps are the same for every template.

Where does my draft go?

Drafts stay in your browser on that device. PDFs are created on our server and not stored.

Your draft is saved in the browser you are using, on that device. There is no account to upload it to. When you download a PDF, the document goes to our server only to create the file and is not kept. Clearing browser data or switching devices leaves the draft behind.

These pages are public, so keep drafts anonymous. Use initials, leave out phone numbers and addresses, and add identifying details on paper or in your clinic's records after you download.

How do clinics use the same documents inside healhab?

healhab clinics keep their own versioned templates in Template Studio, and a clinician reviews every report before it publishes.

healhab is clinic software for therapy and rehabilitation practices. In Template Studio a clinic drafts, publishes, versions, duplicates and archives its own templates, with smart variables that fill patient details. Speech, occupational and behaviour therapy clinics start with built-in document starters. Physiotherapy, child development and rehabilitation clinics author their own during setup.

A report written in healhab does not publish on its own. A clinician reviews it first. Once published, the family can see it in the Family Portal, and the clinic can share it on WhatsApp where sharing is set up and the family has opted in.

How these templates work

healhab's therapy templates give you a ready structure for assessment reports, treatment plans, SOAP and session notes, progress reports, home programmes, discharge summaries, intake forms and data sheets. They cover speech therapy, OT, ABA and behaviour therapy, early intervention, physiotherapy and rehabilitation. Fill one in online without signing up and download a PDF.

You already know what a good report looks like. The slow part is rebuilding the headings, tables and sign-off each time. Here those layouts sit in one place, sorted by your therapy or by the document you need today.

Every template opens in a browser editor and downloads as a PDF. Each page explains what goes in every section, with a short sample entry where one helps.

Questions

Are the therapy templates really free?

Yes. Every template, the report writer and the tools are free to use without signing up. Download as many PDFs as you need for your own clinical work.

Do the templates include sample entries?

Yes. Each template page shows a short, fictional sample entry for most sections, using initials only. Impression and summary sections have guidance only, because that wording belongs to the clinician.

Do the templates include scoring or test items?

No. They give you headings, prompts and tables. Record the name of any test you used and the result in the form its manual gives, and keep the test form in your clinical records.

Can I use them in India with my own assessments?

Yes. The templates were written for Indian clinics first, with room for home languages, family members who attend sessions and school details. They work alongside the assessments your clinic already uses.

Can I download a Word file?

The report writer downloads PDFs only. To change a document later, keep the draft open in the same browser, edit it there and download a fresh PDF.

Can I add my clinic's logo?

Not in the free report writer. Print the PDF on your clinic's letterhead, or set up branding once per clinic inside healhab.

Write every report inside your clinic system.

Keep these as your clinic's own templates, write from reviewed session notes, publish branded PDFs, and keep families updated.
