About these physiotherapy templates
These physiotherapy templates cover a full episode of care: case sheet, assessment form, treatment plan, SOAP note, progress report, home exercise programme, pain and exercise diary, and discharge summary. Each one is free, suits musculoskeletal, neuro and paediatric patients, fills in online without sign-up, and downloads as a PDF.

These formats follow a patient from first visit to discharge: case sheet and assessment, treatment plan, a SOAP note each session, progress reviews and a discharge summary. The HEP and pain diary go home with the patient.
They are built for Indian practice, with floor sitting, stairs and home languages included. Each is structure only: you choose the tests and scales, and every clinical decision stays yours. Physical therapy clinics elsewhere will find the structure familiar.
Questions
Are these physiotherapy templates free?
Yes. Every template can be filled in online in your browser and downloaded as a PDF, with no sign-up. Your drafts stay in your browser.
Do the formats include filled samples?
Yes. Each template page shows a fictional sample entry under its sections, initials only. Assessment and interpretation sections carry guidance only, because that reasoning belongs to the treating physiotherapist.
Which template should I start with?
For a new patient, the case sheet and the physiotherapy assessment form. Most clinics then use the treatment plan, a SOAP note after each session, and a progress report at each review point.
Do the templates include outcome measures or special tests?
No. They have space to record the tests and outcome measures your clinic uses, with each result as its manual reports it, but they include no test items or grading.
Can I change the headings to match my clinic's format?
Yes. Open any template in the free editor, change the headings and tables, and download your version. Many clinics keep one master copy for the whole team.
Do they work for neurological and paediatric physiotherapy?
Yes. The structure works across musculoskeletal, neurological and paediatric caseloads, and each page explains what to adjust, such as the function table for neuro patients.
Where can I find physiotherapy goal examples?
The physiotherapy goal bank has example short- and long-term goals grouped by area, to adapt with your own clinical judgement.
