About these speech therapy templates

Speech therapy templates give each document in an episode of care a steady structure: case history, assessment report, treatment plan, session notes, data sheets, progress reports, a home programme and a discharge summary. Every template here opens in your browser without sign-up, fills in like a form and downloads as a PDF. None contains test items or scoring.

These templates follow a speech and language case from the first phone call to discharge: case history, assessment report, treatment plan, SOAP notes, data sheets, progress reports, a home programme and a discharge summary.

They are written for Indian practice, where a family may use three languages and parents read updates on a phone. Each gives you structure only; the clinical reasoning, test results and decisions stay yours.

Questions

Are these speech therapy templates free to use in my clinic?

Yes. Fill them in online, download a PDF, print them and adapt the headings for your clinic, with no account. Drafts stay in your browser, so download the PDF before you clear browser data.

Which template should I start with for a new referral?

Start with the case history form. Then use the comprehensive assessment report for a full evaluation, or the short speech and fluency report for one concern. The treatment plan follows from whichever report you write.

Do the templates include standardised tests or scoring?

No. They give you places to record any test you used and its result exactly as the manual reports it. Licensed test items and conversion tables belong to the publishers and to your clinical judgement.

Do the templates include sample entries?

Yes. Most sections show a fictional sample entry, using initials only, so you can see the level of detail to aim for. Summary and impression sections have no sample; the treating clinician writes those.

Can I write reports in a language other than English?

Yes. The editor accepts any text, so you can add a parent note in Tamil, Hindi, Marathi or another language beside the English report. Record home languages in the case history too.

What is the difference between the goal sheet and the treatment plan?

The treatment plan is the full document, with baselines, success criteria, guidance and a review plan. The one-page goal sheet puts each communication area in one table for teachers and the team.

Do you have speech therapy goal examples to use with these templates?

Yes. The speech therapy goal examples page groups measurable goals by skill area, such as speech sounds and fluency. Treat each as a starting point and rewrite it from your own assessment.
