Behaviour therapy documents work as a chain. The assessment and FBA describe what a person can do and why behaviours happen. The BIP and treatment plan set goals, and notes, data sheets and reports track what changes.
Every template is structure only, in Indian English, with sample entries that show the level of detail to aim for. Nothing in them rates, labels or interprets a person. Clinical judgement stays with the therapist who signs.
Questions
Is behaviour therapy the same as ABA?
The terms overlap. In India, “behaviour therapy” is the common service name for structured support with skills and behaviours of concern, and many centres use methods drawn from applied behaviour analysis (ABA). These templates work for either, under whatever supervision arrangement your clinic follows.
Which template should I start with?
Start with the assessment report for a new referral. If one behaviour is the main concern, add the functional behaviour assessment. The BIP and treatment plan come next, then session notes and data sheets for day-to-day work.
Are the data sheets printable?
Yes. Each data sheet opens in the editor, where you can add rows or change column labels, and downloads as a PDF you can print for the session room or send home with a caregiver.
Do the templates include any assessment tools or scoring?
No. They contain no test items, curriculum checklists or rating systems. If your clinic uses a licensed tool, record its name and the results as its manual reports them in the relevant section.
Can families read these documents?
The home plan is written for parents and caregivers. The other documents are clinical records; share them with families in plain language, and with schools only with the family's consent.
Write every report inside your clinic system.
Keep these as your clinic's own templates, write from reviewed session notes, publish branded PDFs, and keep families updated.
