Therapy progress report format and sample

A therapy progress report format with sample goal entries that put baseline and current data side by side.

Progress reports by therapy

The essentials

What goes in a therapy progress report?

Period, attendance, progress on each goal, what helped, barriers, home practice and next steps.

  • Period covered: start and end dates.
  • Attendance: sessions offered and attended, and anything that interrupted therapy.
  • Goal-by-goal progress: baseline, current measure and a status such as achieved, progressing, not yet progressing or discontinued.
  • What helped: strategies, supports or routines that made a difference.
  • Barriers: what made progress harder, described without blame.
  • Home practice: what the family did between sessions and how it went.

How do you write progress against a goal?

Put baseline and current numbers side by side, then say what changed in daily life.

For a fictional child, M., working on requests: "Baseline (June): requested a preferred item with a single word in 2 of 10 opportunities with a verbal model. Current (September): 7 of 10 without a model at snack and play. Mother reports M. now asks for water at home." Give the status after the evidence.

If a goal did not move, say so and explain what you will change. A plain account of slow progress helps a family more than a vague sentence.

How do progress reports differ by therapy?

Speech and OT group goals by area, ABA adds graphs, physio compares measures over time.

Speech and OT reports group goals by area and comment on use at home and school. ABA reports often add graphs or tables of target and behaviour data. Early intervention reports follow routines and family priorities. Physiotherapy reports compare measures such as range, strength or walking distance. Rehabilitation reports combine each discipline into one summary.

Progress report, SOAP note or discharge summary?

One session, a period of weeks, or the whole episode of care.

  • SOAP or session note: one session, written the same day, for the clinical record.
  • Progress report: weeks or months, for the family and other professionals, comparing current performance with baseline.
  • Discharge summary: the whole episode of care, written when therapy ends, with recommendations for afterwards.

These three documents are easy to confuse because they all report what happened.

Keep page one short, explain abbreviations, give counts behind percentages, and add a note for teachers.

Parents often share the report with grandparents, teachers or a paediatrician as a PDF on their phone. Keep the first page short and plain, explain each abbreviation once, and give the counts behind any percentage. For school-age children, a short section on what the teacher can do in class helps at the next school meeting.

About progress reports

A therapy progress report summarises what changed over a period of therapy. It states the dates covered and sessions attended, reports each goal against its baseline with current data and a status, notes what helped and what got in the way, and sets out next steps. The templates below give that structure for each therapy.

Families wait for progress reports. Schools ask for them before meetings, and doctors read them to see whether a referral is working. It is where your session notes become a story someone else can follow.

Whatever you call it, the report works best when every statement of progress points back to a goal in the plan and data in your notes.

Questions

Is there a filled sample of a progress report?

Each therapy template page shows a fictional sample entry for each goal section, using initials only. The overall summary has guidance only, because that judgement belongs to the treating clinician.

How often should a therapy progress report be written?

Many clinics write one every three months, at the end of a school term or when the plan is reviewed. Follow your clinic's schedule or any school or funder requirement.

Can I download a progress report as a PDF?

Yes. Fill in the template online and download the PDF, or download a blank copy to fill in by hand.

What if a goal was not worked on in this period?

Say so and give the reason, such as a change in priorities or missed sessions. Mark the goal as not addressed rather than leaving it out.

Should progress reports include graphs?

Graphs help when you have regular data across many sessions, as in ABA. For most speech, OT and physiotherapy reports, a small table of baseline and current measures is enough.

How do I keep progress reports consistent across therapists?

Use the same template, the same status words and the same way of writing baselines. Agree those choices as a team before the next reporting period.
