Printable ABA and therapy data sheets for trials, prompts, frequency, duration, intervals and ABC data, free to fill in or print.

Data sheets by therapy

The essentials

Which data sheet should you use?

Start from the question: accuracy, help needed, how often, how long, or what came before and after.

  • Trial-by-trial (discrete trial): did the person respond correctly on each opportunity? Correct, incorrect or prompted.
  • Prompt level: how much help was needed, from full physical to independent?
  • Frequency: how often does it happen? Tally occurrences in a set time to get a rate.
  • Duration: how long does it last? Start and end times of each episode.
  • Latency: how long before the person starts? Time from instruction to response.
  • Interval: did it happen in each interval? Partial, whole or momentary methods.

How do you record trial-by-trial data well?

Define correct and prompted first, record each trial at once, and report counts with percentages.

Define exactly what counts as correct and as prompted before you start. Record each trial straight away. Keep prompted trials apart from independent ones, because correct with help and correct alone are different points.

Summarise with counts as well as the percentage: "7 of 10 correct, 2 prompted, 1 incorrect" says more than "70%".

How do you write good ABC data?

Write what you saw and heard, in order, and leave out guesses about why.

For a fictional child, S.: "A: Teacher said 'tidy-up time' and took the puzzle. B: S. lay on the floor and cried for about 2 minutes. C: Teacher gave the puzzle back for one more piece." Patterns across many entries support a later hypothesis, which belongs in the functional behaviour assessment.

Data sheets in speech therapy, OT and physiotherapy

Trial sheets for sound targets, task analysis for self-care, exercise logs for physio.

Speech therapists use trial sheets for sound or vocabulary targets with the cue level noted. OTs use task analysis for dressing or handwashing and duration sheets for time on task. Physiotherapists use exercise logs with sets, repetitions and reported pain. In each case, record the same thing the same way every time.

Paper data sheets or a live tracker?

Paper is simple and easy to file. A phone tracker adds up totals for you.

Paper sheets are visible to everyone in the room, easy to file, and parents can use them at home. Many Indian clinics prefer them. healhab's session data tracker counts trials, prompts, frequency and duration in the browser and copies a summary you can paste into a SOAP or session note.

About data sheets

Therapy data sheets are blank recording grids for collecting session data the same way every time. Common types are trial-by-trial (discrete trial) sheets, prompt-level sheets, frequency and rate sheets, duration sheets, interval recording sheets, ABC sheets and task analysis sheets. The templates below can be filled in online or printed for paper data collection.

Data is how you know a goal is moving. A good data sheet is quick enough to use mid-session and consistent enough that two therapists recording the same target get comparable numbers.

ABA services use them most, but speech therapists, OTs and physiotherapists use them too. Pick the sheet that matches the question you are asking.

Questions

What is a trial-by-trial data sheet?

A sheet that records the result of each planned teaching trial for a target, usually as correct, incorrect or prompted, so accuracy and prompt dependence can be tracked across sessions.

What is an ABC data sheet?

A sheet for recording antecedent, behaviour and consequence each time a behaviour of concern happens. It is a standard part of a functional behaviour assessment.

Is there a sample of a filled data sheet?

Each sheet page shows a short fictional sample entry for most sections, using initials only. The sheets record observations; any interpretation of the pattern is left for the clinician.

Can parents use these data sheets at home?

Yes, with a simple target and a short explanation. Frequency tallies and task analysis checklists are usually the easiest for families to keep.

Are these ABA data sheets free to print?

Yes. Open any sheet, fill it in online or download it blank as a PDF and print as many copies as you need.

How do I turn data sheets into a progress report?

Summarise each target across the period with the first and latest data, give the counts behind any percentage, and describe changes in prompt level as well as accuracy.
