Home programmes by therapy

The essentials

What goes in a home programme?

Purpose, a few activities with dose and materials, warning signs, a practice log and a review date.

  • Purpose: one sentence linked to a goal the family recognises.
  • Activities: what to do, step by step, with a drawing or photo where it helps.
  • Dose: how many times, how long and how often.
  • Materials: things already at home, such as a steel tumbler, a towel or a step.
  • Watch for: signs an activity is too hard or should stop, and who to contact.
  • Practice log: a simple table to tick each day.

How do home programmes differ by therapy?

Physio lists exercises and safety notes; other therapies build practice into daily routines.

Physiotherapy programmes list exercises with sets, repetitions, holds and safety notes. Speech practice works best inside routines such as mealtimes or bath time, in the home language. OT programmes place fine motor, self-care or regulation activities at set points in the day.

ABA home plans focus on adults setting up, prompting and responding the same way as at the centre. Early intervention coaches parents within routines. Rehabilitation programmes often include carers and equipment.

How do you write a home programme a family will use?

One instruction per line, starting with a verb, and only the two or three activities that matter most.

Write one instruction per line, starting with a verb: "Sit on a firm chair with feet flat". Use the family's words. Limit it to the two or three activities that matter most this month. Show each activity in the session and watch the family try it before you send the sheet home.

Ask who will actually do it. In a joint family that may be a grandparent, so write it for them to follow alone.

Translations and sharing

Write in the language the family reads best, and send one clear PDF that reads well on a phone.

If the family reads another language more comfortably, write in that language and keep counts in numerals. Many families receive and forward programmes on WhatsApp. A PDF with large text and one activity per section reads far better on a phone than a photo of a handwritten sheet.

Home programme or home exercise plan in the report?

Keep it separate when activities change often or someone other than the parent does them.

Some clinics put home activities inside the progress report or discharge summary. A separate programme works better when activities change every few weeks or someone other than the parent carries them out, because you can update and reshare it without reissuing the report.

About home programmes

A home programme tells a family or patient what to practise between therapy sessions: each activity, how to do it, how often and for how long, what to watch for, and how to record practice. Physiotherapists call it a home exercise programme. The templates below give versions for physiotherapy, speech therapy, OT, ABA and early intervention.

Most of a person's week happens outside therapy. A home programme carries the session into the kitchen, the classroom and the evening walk, so a family must be able to read it once and act.

The US spelling is home exercise program, or HEP. These templates print well or go out as a PDF that opens on a phone.

Questions

Is there a sample home exercise programme?

Each therapy template page shows fictional sample activities with dose and safety notes, using initials only. Choose and adjust activities for each person from your own assessment.

What is a home exercise programme?

A written set of exercises a physiotherapist gives a patient to do between sessions, with dose, technique and safety notes. Other therapies use the broader term home programme.

How many activities should a home programme include?

Usually two to four. Fewer, well-practised activities tend to get done; long lists are often set aside.

Can I add pictures to the home programme template?

The report writer handles text and tables. Many therapists add simple drawings on the printed PDF or send a short video separately through the clinic's usual channel.

How should families record home practice?

A tick box for each day is enough for most families. Ask for one short note when something goes well or badly, and review the log at the next session.

Can I write the programme in Hindi or another language?

Yes. Type into the template in any language your keyboard supports, then download the PDF.
